A Primary Focus of Our Services: Care Management | Client Advocacy

In our last blog, “Caring for My Father: Top Ten Lessons Learned,” we introduced the primary lessons learned during our 6½-year caregiving experience with my father. Among those lessons, we believe it’s important to highlight the lessons related to actively monitoring and managing care to ensure optimal outcomes and satisfaction. In essence, we are highlighting the true need for and benefit of care management and client advocacy.

Our overarching goal of caregiving and personal assistance is to contribute to the overall medical health of our clients and keep them in their home for as long as practical – regardless of where our clients call home. To accomplish this goal, our personal assistants’ focus on the client relationship, suitable diet, medication assistance, ambulation and mental stimulation. By working with our clients, clients’ families and their health care team another important objective of optimal home care services can be accomplished: preventing hospital admissions and readmissions.

With the levels of care offered: Companion, Basic and Specialty, coupled with our care management, our clients can have an all-encompassing experience resulting in optimal care, high satisfaction, and lower costs. Primary aspects of our care management included:

  • Assessment & monitoring of care
  • Holistic, client-centered, goal-oriented education & advocacy
  • Advocate for clients and families
  • Liaison between clients, families and service providers
  • Coaching family caregivers

Our personal assistants can provide long term care and assistance versus the short-term, episodic care provided by skilled nursing facilities and home health. Recent reports indicate that home care has reduced doctor’s visits by as much as 25 percent. In terms of hospital readmissions, HomeWell Senior Care’s clients have experienced hospital readmission rates as low as 8 percent which is 12 percent lower than the industry standard.
